Student Enrollment Requirements

To enroll as a student, please prepare the following documents and ensure that all required forms are completed.

1. Personal information

Birth Certificate (original and photocopy)
2x2 Recent ID Photos (at least 4 copies) within the last 6 months
Valid ID (Government or School ID)

2. academic records

Report Card / Transcript of Records (original and photocopy)
Certificate of Good Moral Character from the previous school admission
Entrance Examination Results (if applicable)

3. Health and safety requirements

Medical Certificate issued within the last 6 months, indicating that the student is in good health and fit to attend classes. The certificate must be signed by a licensed physician and include:

○ General health status

○ Immunization history

○ Any existing medical conditions or allergies

Vaccination Record (Original or photocopy, if applicable)
Dental Certificate (Optional but recommended)
